¹ A per­son suf­fer­ing harm may sub­mit a writ­ten de­clar­a­tion in writ­ing or make the de­clar­a­tion or­ally on re­cord.

² In the de­clar­a­tion the per­son suf­fer­ing harm may do either or both of the fol­low­ing:

a.

re­quest the pro­sec­u­tion and pun­ish­ment of the per­son re­spons­ible for the of­fence (a crim­in­al com­plaint);

b.

file private law claims based on the of­fence (a civil claim).
