¹ In­form­a­tion Re­quest Type IR_12_TEL com­prises the fol­low­ing data about tele­phony and mul­ti­me­dia ser­vices:

a.

if avail­able, the unique sub­scriber iden­ti­fi­er (e.g. cus­tom­er num­ber);

b.

the fol­low­ing data about the re­ques­ted ser­vices and any ad­di­tion­al as­so­ci­ated tele­phony and mul­ti­me­dia ser­vices:

1.

if ap­plic­able, pub­lic ad­dress­ing re­sources (e.g. MS­ISDN, SIP URI, TEL URI) and private ad­dress­ing re­sources (e.g. IMPI) as­so­ci­ated with the ser­vice con­cerned and their term of valid­ity,

2.

the unique device iden­ti­fi­ers in ac­cord­ance with in­ter­na­tion­al stand­ards (e.g. PEI) of the devices used in con­nec­tion with the ser­vice con­cerned from the pro­vider in the last 6 months and, if avail­able, the names of the devices,

3.

if ap­plic­able, the IC­CID of all SIMs as­so­ci­ated with the ser­vice con­cerned and their term of valid­ity, the PUK and PUK2 codes, the IM­SI or the SUPI, the MS­ISDN or the GP­SI and the eUICC-ID,

4.

in the case of a multi-device of­fer: in­form­a­tion on wheth­er the device is the main device or a sec­ond­ary device.

² The re­quest for in­form­a­tion shall spe­cify the peri­od to which the re­quest relates. It shall con­tain at least one of the fol­low­ing query cri­ter­ia:

a.

the pub­lic ad­dress­ing re­source (e.g. SIP URI, MS­ISDN, GP­SI);

b.

the IM­SI or the SUPI;

c.

the unique device iden­ti­fi­er in ac­cord­ance with in­ter­na­tion­al stand­ards (e.g. IMEI, PEI, MAC ad­dress);

d.

the in­stall­a­tion ad­dress of the fixed loc­a­tion ac­cess to the net­work;

e.

the private ad­dress­ing re­source (e.g. IMPI);

f.

the IC­CID.
